Where to call for an appointment:
If you are ill, or in urgent need of care, an appointment will be scheduled for you as soon as possible. In emergencies, you may be seen immediately at one of our offices, or at an agreed upon hospital location. If you would like to schedule a less urgent appointment, please call as far in advance as possible to schedule a time convenient for you. Our receptionists will assist you with transferring necessary medical records, x-rays, test results, etc., from your personal physician. Please be prompt for appointments. Your appointment time is reserved especially for you. Arriving promptly for your appointment helps minimize waiting time for you and other patients. Please notify us as soon as possible if you must cancel your appointment, or be unavoidably late. If you miss an appointment without cancellation or advance notice, you may be charged for the time.
Your participation and cooperation are very important to us as we strive to bring you quality medical care. We welcome and encourage your comments and suggestions. It is very important that problems be resolved quickly and satisfactorily. If you have any questions or concerns about your care or about The Minneapolis Clinic of Neurology, please discuss them with a staff member or call our Administrative Offices at (763) 588-0661. If you need a prescription refilled, please contact your pharmacy. Please do not wait until you are completely out of your medication before requesting a refill. Your pharmacist may need to call our physician, or in the case of a controlled substance, a written prescription may need to be sent to the pharmacy. Your physician may wish to see you prior to authorizing refills. We will not be able to prescribe medications for you unless you have been seen by one of our physicians. Please notify us immediately of any unexpected changes in your condition that you feel may be caused by your medication.
As neurologists and allied health professionals, we are particularly sensitive to your needs for confidentiality and privacy. All information in your medical record is confidential and cannot be released to, or discussed with, another person without specific written consent from you or an authorized representative. Please ask your physician which hospital you should use in case of emergencies. If you need to be hospitalized for scheduled care, you and your physician can discuss the most appropriate and convenient facility and location. Life-threatening: Call 911. Go with responding medical team to the hospital or emergency medical facility. Call your physician as soon as possible thereafter. Urgent care: During business hours, please call your physician for immediate advice and directions. After hours care: Please call (763) 588-0661. We maintain 24-hour "on call" medical care. Our answering service will contact a physician who will return your call promptly.

Q: Do I need to arrive earlier than my appointment time? A: If this is your first visit to our Clinic, please arrive 10 - 15 minutes prior to your scheduled appointment time to register. Registration includes collection of demographic (address, telephone number, etc.) and insurance information, as well as making a co-payment, if required by your insurance company. Even if you have pre-registered by telephone, we ask that you arrive early to sign appropriate forms and/or complete our medical history forms. For follow-up visits, please allow enough time to verify information and make any necessary changes, such as address, insurance, etc., as well as to make your co-payment, if applicable. Q: Do I need a referral to see a neurologist? A: Yes. A referral from a medical professional is necessary to ensure that you are being seen by a specialist who is experienced in dealing with your specific condition, and that you are seen in a timeframe appropriate for your medical condition. Please note that a referral from your medical professional is different than an authorization from your insurance company.

It is your responsibility to determine if we are participating providers in your plan. Many changes have taken place in the health insurance industry in recent years. Since every plan is different, be sure to check your coverage if you have specific questions about what your insurance will pay. Your insurance company requires that you present your insurance card at each and every visit. Copayments, which are usually listed on your insurance card, are due at the time of service. We are required by your insurance carrier to collect co-payments. If you do not pay your co-payment at the time of service, a $20.00 service charge will be applied to your account to cover the costs of billing and processing. Your co-payment could increase without a new card being issued to you. Your insurance company can tell you what your co-payment is. We will mail you a statement of your account, which is due upon receipt. Your insurance company determines any balance due. Many people have policies with only partial coverage. This may mean that you will pay more out of your own pocket than you have in the past. We accept cash, checks, money orders, debit cards and all major credit cards. A 1.5% monthly finance charge will be added to all accounts with any remaining balances over 30 days old. Limited financial arrangements may be made only by speaking to one of our Patient Financial Services Representatives. While we make every effort to resolve insurance issues, please remember that you are ultimately responsible for the cost of your health care. If you need to cancel your appointment, please notify our office 24 hours before your appointment (please contact us on Friday for Monday appointments). Receiving cancellation information in advance allows us to schedule and serve other patients. We reserve the right to bill you $50 for missed appointments without a 24-hour advance notice of any appointment change or reschedule. This fee is not covered by insurance carriers or Medicare and will be your responsibility to pay before your next visit. Our physicians may charge for completion of forms, for which they may request a pre-payment at the time the form is received. Our fees are available upon request. CASH PLAN: If you do not have insurance, or have an insurance plan with which we are not contracted, we can provide you with a 20% discount for payment on the same day the services are provided. This discount also applies to credit card payments. This 20% discount is only offered on cash and credit card payments made on the day the services are provided. Financial arrangements may be made only by speaking to an MCN Patient Financial Services Representative. FREQUENTLY ASKED QUESTIONS ABOUT PATIENT ACCOUNTS Will you bill my insurance company? Yes. We will bill your insurance company directly. We are contracted with most plans and take any discounts that are part of our contracts with them. If we are not contracted with your particular insurance company, we are not obligated to take a discount from your plan. How much is my co-payment? Can you bill me for it? Co-payment amounts are usually listed on your insurance card; however, your co-payment could increase without a new card being issued to you. Your insurance company can tell you what your co-payment is. Co-payments are due at the time services are rendered. We are obligated to collect this amount as directed by your insurance carrier. if your co-payment is not paid at the time of service, a $20.00 service charge will be applied to your account to cover the costs of billing and processing. What is a deductible? A deductible is an amount that you are required to pay out of your own pocket up to a certain limit before a higher level of insurance benefits is available to you. Although not everyone has a deductible policy, they are becoming more common. A deductible can be as small as $50 or as large as $5,000 or more. I have insurance coverage – why did I get a bill? Please review any bill you receive from us carefully. If you have questions, please call your insurance company or us for assistance. Many people have policies today with only partial coverage. Please bring the following information to your appointment: date of accident, name of your employer, workers' compensation claim number and name and address of the insurance company. Please provide us with information regarding other health insurance coverage you carry, should your Worker's Compensation claim be denied.
